I'm probably a dissenting voice for these, but I'll go with Crystal Palace, Chef Mickey's and Liberty Tree- all for dinner. (Can't comment on breakfast because I'm not a breakfast person.) We did CP and CM last week and I was really disappointed with both.

1. Kona
2. Whispering Canyon Cafe (I'd give them another chance, but I'd have a lynch mob on me from the rest of my party if I did)
3. MGM Pizza Planet

I've had OK food at other places, and there are places that I wouldn't rush back to, but these are the three that I'll never be at again.

Narcoosee's (unless I have it on good authority that they've improved!).

Yacht Club Galley (just had a name change, which slips my mind. The single worst meal and service ever. A CM server actually shouted at her table! I mean, bent over at the waist, face red shouting)!

Not huge fans of 'Ohana, Whispering Canyon, Planet Hollywood, Rainforest Cafe or Bongos. Can't say we'd never go again, but wouldn't jump to it real fast.

Good question. I think there was a poll about this some time ago.

Before I say our do not return, first a disclaimer. My wife and I visit WDW frequently (~4 visits a year) and enjoy a more up-scale experience but we are also happy with pub food type of places. We value good service highly. We recognize that Disney restaurants seem to go through phases where one is up and an old favorite is on the down-slide. With so many places to eat at WDW it doesn't make sense to go to a place that we've had bad experiences at - even if they have been improving. So, the list?

Fultons - poor service, food just OK.
Big River Grille & Brew Works - Very poor service, food often not prepared properly
Mexico - OK service, OK food, overpriced
China - can get better right here at home
Italy - better food and service right here at home
Rain Forest Cafe (either one) - poor service, food just average, atmosphere way over-done
Planet Hollywood - very poor service, average food, overpriced

As much as this goes against the grain of most - Crystal Palace - we had a terrible experience there with out daughter. She was passed by by nearly all characters; poor timing I guess, which really upset her. Also, it was perhaps the loudest most obnoxious breakfast we had ever been to. Even our 5 year old complained, LOL.

Other than that, and the Rainforest Cafe, we have always had a good experience.

For us they are:

Boma
Cape May
Garden Grill
Chef Mickey
Mama Melrose

Mrs. Disney Ron

1) Crystal Palace- Never again for lunch/dinner... but the breakfast is very good. Food is good with a wide variety and character interaction was great
2) Mama Melrose-We have been twice (because we always give a place a second chance)... both times the food was rubbery, oily, sause was brownish is color and too many spices that made it taste terrible. Also there was a considerable wait both times even though we did have reservations. Not worth the wait or the money (imo). Sad because it is part of the Fatasmic package.

Planet Hollywood
Liberty Town Tavern (dinner -might have to try it for lunch)
Hollywood & Vine
Commissary at MGM

1900 Park Fare--Since the rehab the selection just doesn't cut it for a basic meat and potatoes kind of family. While the character interaction was wonderful, I can't justify spending over $100 for a family of 3 that left hungry.

Coral Reef--This used to be one of our very favorite restaurants, but the inconsistency of the meal, a few bad servers, and having to wait a ridiculously long time to be seated regardless of what time we made our ADRs has made us drop this place from our list.

Crystal Palace...just plain yuck!
Planet Hollywood...overpriced for mediocre food & wayyyyy too loud!
Sci-Fi...OK for one time just would not repeat the experience.

Chefs De France - Poor service (Server had other places he wanted to be and we were obviously keeping him from where ever that was) and food was mediocre at best.

We have eat here only once because we were so disappointed that we just don't care to go back.

LLT (if I want microwave food I can fix it at home)

50's Prime Time (food was undercooked and no taste) seating was just disappointing I just don't want to eat with families I don't know.

Tony's
Hollywood and Vine

We thought both meals were terrible there. We even gave Tony's a second chance on a different trip - still terrible.

Another one that was nice to try once, but not overly impressed with was Boma. Food was okay - just not worth the travelling distance to us.

DH will never return to CHefs de France either. We had a really bad experience there, expecially the service. I would however give it another try just to see if we just had a bad night.

Sorry to say, but Chefs de France. It was chaotic and the food was just okay. It seems many people really like it, so we will give it another shot someday.

ABC Commissary in the Disney Studios - just gross
Sci Fi Dine-in Theatre - also gross, but awesome atmosphere (kids love it)
Planet Hollywood - gross
Rainforest Cafe - gross

I can honestly say that there isn't a resturant I would not go back to. There are a couple that we've been disappointed in and have decided to wait a couple of trips before we go again.
