Are non rev passengers always airline employees or are passengers flying for free using their FF miles considered to be non rev passengers, too?

No, using FF miles would not be non-rev. Non-rev passengers have to standby to get on flights unless it's part of their job. FF miles tickets are treated like regular tickets.

Passes issued to non-employee passengers (not using award tickets) are also non-rev.
