No more special services form on DCL with food allergies??

We haven't cruised in a while, but will be boarding the Disney Magic in a few months (can't wait!! :banana: ) . In the past I always needed to fill out a special services information form and notify DCL of my son's peanut and tree nut allergy ahead of time. I just downloaded the form and there isn't a box for Food Allergies - it actually says to bring any food allergies to the attention of the Head Server prior to ordering. That's it?? It seems sort of lax (although I'm sure that they are beyond prepared for multiple people with multiple allergies as food allergies are common).
 
At least back to 2010, the Special Services form for DCL has been more of a checklist of medical issues (oxygen, medications, need for communication kit, etc.) without space to add "notes" for other needs. Very limited use for the form, in my opinion. I suggest just calling to have a note added to your reservation -- call the Special Services number (407-566-3500) rather than the main reservations number. Then when you board you can go to the dining change location and check to make sure the allergies are noted. The Head Server will go over options with you, possibly have you order for the next day after dinner each evening.
