When you push down on the hitch latch it lifts up a "fork" that locks around the ball. If that is loose it can hop off the hitch ball or if the ball is the wrong size it can hop off the hitch ball. Have towed my whole life never had a trailer come off the ball. Have seen it happen but usually with UHaul trailers where the driver knows nothing about trailers and hitches.
You may suggest he run the van/trailer by the local camping store/trailer sales "place" and have them give the trailer and van a look over before their next and each trip. To not do the proper preventative maintenance before a trip is not a good idea especially with kids in the vehicle. If it had been a big trailer or a small tow vehicle it could have "tugged" the van off the road into a ditch or tree at speed...
I do wish them the best of luck but while in Orland he needs to run the whole rig by a camping world and have them give it a look over.
