Need to get my Mother a new birth certificate copy, born in Quebec

Complicated story but here goes. My brother is surprising my Mom with a trip with him and his family to Dominican in January. My mother will need a passport (she's never had one in her life!) To get a passport, of course, she needs a birth certificate. She has her original copy. She was born in Quebec.
The first problem is, Quebec birth certificates dated 1960 something and prior are NO LONGER ACCEPTED!! WHY? I have no clue. Anyway, she thus needs to get a new copy. This has not been easy to figure out how to do.
She has lived in Ontario since the age of 6?
We have gone to the website http://www.etatcivil.gouv.qc.ca/en/services.html
We use the online service as she wants this expidited and does not want to have to mail documents in.
Quebec has this process whereby you HAVE to Register for a "Quebec Government Authentication Service, User ID. Ok, so we start that process as you can't get the other application parts till you have registered for THIS service. However What it wants for info to register are:
1. Her SIN number (no problem, we have that)
2.Her notice of assessment number with Quebec Revenue
3.Income entered on line 199 of her income tax filed with Quebec Revenue.

We have a problem with numbers 2 and 3. Her notice of assessment has a 12 digit number, not an eleven digit one.
There is no line 199 on her ONTARIO income tax form that she filed. AND This system will not NOT recognize her SIN number!

Please can someone help me figure out how to get her a new birth certificate? I recently got a new copy for my daughter, born in ONtario, and never was it this complicated.
 
The only way to apply in person is to drive to Quebec City, possibly Montreal too, but either way, both are too far.

It looks like the only option for her is to fill out the form and mail it in, and I checked the ID requirements. She doesn't have to send actual documents, a photo copy will suffice. Mailing will take longer but ce la vie.
Does anyone understand why her ORIGINAL Birth Certificate is no longer valid?
 
Hi -- I got caught in this one as well. The reason the old birth certificates are no longer valid is that they have no security features that prevent them from being copied/forged. After 9/11, they decided it was way too easy to get a fake Canadian passport using these bits of paper from various hospitals, or baptismal certificates (which also used to be allowed for passport purposes).

I don't recall getting an online ID number to get my birth cert from the QC gov't but I did download the form, filled it in and sent it off with the appropriate photocopies. It did not take that long if I recall correctly!

You can even pick up copies of the forms at the "Caisse Desjardins", of which there are a few in Ottawa and I'm sure some over in Gatineau.

Having been born in Quebec, I understand exactly what you are going through. You can do it over the phone, and will have more success if you, or someone you know, speak french when talking to them. .. that seems to help alot. Also, Quebec has several times update their requirements re birth certificates. The orginal I had in 1964 was invalid by the 80's, and I needed an updated one when I had my first child and wanted to collect EI. Then when I had another child in 2004, I needed another one because the one from the 80's was no longer valid!:scared1: It must be the Quebec gov'ts way of making money:mad: Good luck!
 
Thanks for the info. I called my mom as she was filling in her application form. Her "Birth Certificate" does not actually say "Birth Cetfificate", It is dated 1942, has a registration number on it and a seal but it was issued by a Church. So we think it is actually a Baptismal Certificate which we now know, years ago was able to be used in place of a birth certificate. Suffice to say, back in the 40's, 50's and 60's, since the baptismal certificate could be used in place of and the same as a birth certificate, many people did not get birth certificates for their babies. It's a good thing my brother didn't wait till Christmas to surprise her with this trip for Mid January. She would never have been able to get all her documentation in place before the trip. I'm glad my brother is taking her though, she has NEVER been out of the country except for one trip to the Northern U.S. I wished we could take her on our Disney Cruise in 4 weeks, but we have no more seats in our Van (driving down). Maybe I'll take her on a Mom and Daughter Cruise next year.
 
Requested my Quebec birth certificate a few years ago. Spoke with the bureau directly things we smoothly. Filed my request by mail. No problems

You can apply in person in Quebec City or Montreal. I would call firs to make ensure that all my documents were in order.

I also had to apply for a Quebec BC in 2002 (when they changed the requirements for passport applications from those born in Quebec). Prior to that, I had no BC (I used my baptismal certificate).

I did it from Ontario very easily (so easily, that I don't actually remember what I did). I know that it didn't involve any travel to Quebec or needing to be fluent in French (I don't actually recall any speaking in French, or even a phone call). I just filled in a form and mailed it in. I'll look through my files and see if I still have a copy of the form.
 
Thank you everyone for all the info. My mother filled out the form last night, got her photocopies today, and has just now sent if off by courier. DONE! They will receive it by 3pm tomorrow, so hopefully by paying the expidited delivery fee, she will have her BC by the end of next week. If only I had asked this question here a few days ago, it could have saved us the hassel of trying to figure it out through their online services! LOL

Ditto. It actually went so smoothly that I don't remember much about it.
 
I'm glad everything got resolved. Don't be afraid to call the government office next time. I had to call a bunch of times when sorting out my parental leave, and the people on the phone were a huge help. I always spoke in English too and it wasn't a problem.
