need t-shirt help please

HELLO
I just unsuccessfully tried to iron on a name design (from Julia&Nicks mom- thanks!). The top of the transfer is cloudy white. The bottom is flaky. I left the paper on until it cooled, the directions said you can remove it either hot or cold. The color of the design looks good. I used light paper with a light blue shirt. Any help would be greatly appreciated!!!
