Need some guidance from an attorney.

Here's the scoop.

My girlfriend and I (both late 20's) are taking my two little sisters to WDW next month. My sisters are age 17 and age 9. We want to prepare a "legal guardian" document to take with us on the trip in the event something should happen and we need to authorize medical treatment.

If there is a kind-hearted attorney out there - could you please give me an idea of what a form like this needs to include. I'm not asking for it to be written - but can you give me the basic info that needs to be included in it?

I hate to ask for free legal advice - but honestly we're trying to be nice by including my sisters in our trip. We really didn't budget on paying an attorney several hundred bucks for this. This was kind of an afterthought.

You don't need an attorney. I can tell you from personal experience that you need a noterized statement of permission from the parents and an insurance card (or copy). You will also need each girl's SS#.
 
Don't forget to include any allergies the girls might have or any medication they might be on.

Yep, that's all you need. Good advice to take along a list of allergies too. HAVE A GREAT TIME! What a nice big brother you are!
 

Just thought of something else too. You should have the name, address and phone number of their doctors. In an emergency they might not remember who their dr. is.
 
you might check with your local hospital, I know ours has sheets out that you can pick-up and fill out with everything you need to know about medical care when others have your children. I know we use to use the sheets for sitters.
 
We we went to CA w/o kids we got "Parental Authorization For Care and Treatment of Minor Dependent" forms from their doctor. They required it to be from their office. We needed 2 people to sign and print name. We copied the front/back of insurance card & stapled to papers.
We didn't need a notary.

But did you have to use the form?

Was at WDW in June with my uncle who had his 2 year old granddaughter with him. She got a bad sinus infection and the walk-in clinic wouldn't treat her because the form he had wasn't notarized. They had to go to the emergency room at the hospital for treatment because of this. I'd play it safe and get it notarized.

We have done similar statements when both DH and I have gone out of town and left our kids with babysitters here at home.

We did not have them notarized.

We also listed the kids' primary care doctors and dentists, and any allergies/meds they were on.

And we listed where we could be reached, in the event that phone consent was needed.
