Need help with teacher gift book

This Friday my DD's class has a teacher appreciation lunch so I am going to be in with the class. I've already started the teacher's scrapbook but wanted to have each child in the class have an individual page. (I'm going to ask the children to bring in 1-2 pics of themselves, etc.)

My question is for anyone that has ever done this before...what kind of questions did you have the kids fill out?

Like what they learned from the teacher? etc.... Any help would be appreciated.

I just did this today!

Here are some questions to use:

What do you want to be when you grow up?

What is something Mrs. X says all the time?

What was the coolest thing you learned in Kindergarten?

What do you like best about Mrs. X?

What should Mrs. X do with all her time off in the summer?

What will you miss most about Kindergarten?

What is your favorite thing to do in school?

What does Mrs X eat for lunch?

When Mrs X isn't teaching, she likes to....

I just finished making a scrapbook for my daughter's kindergarten teachers. I was lucky enough to get to go to school and hlep with the Valentine party and took the opportunity to photograph each kid while I was there.

I made one book and had the pages copied for the assistant teacher.

The copies turned out very well and I just used white card stock instead of photo paper. The only problem with the copying was a small border around the copied page made each of them about 1/4" smaller than the original page all the way around. So, I trimmed that border off and just mounted each page on coordinating card stock that was cut to fit the 8 1/2 X 11 for each page. Then, I made all new tags for the copied book to match the orginal book's tags. All of the tags are easily removable from the pages, so they can journal on them and place them right back. It really turned out great and gave a little more substance to the copied book. I am an embellishment fanatic and the embellishments really look 3-d on the copied pages!! You actually have to touch it to know they are not real.

I made one page for each kid and I made journaling blocks or removable journaling tags for each that I left blank so that the teachers could make their own comments about each child. I also used the loose leaf notebook scrapbook paper for the last page so that she could have each child sign their name or whatever they want on the back. I used the 8 1/2 X 11 CM books. I am going to insert a few photo sleeve pages in the back of each book as well so that if they want to include any writings or art work of the children in their books they will be able to easily do so.

It was great fun and I'm giving their books to them tomorrow!
