need etiquette help w/ proper place settings

what side of the plate does the glass go on?
What side do the forks go on?
The knife and spoon are on the other side--which is on the outside?
Obviously my beer and chips parties haven't prepared me for this!

Hmmmmmmmmmmmmm...I never knew there was a "formal" and a "casual" place setting like in the following link. Too many pieces would confuse me! Keep it simple!! Apparently, the casual is what I always use. Salad fork on the outside, working your way to the inside.

At my house you are lucky if all the silverware matches! I have never owned enough place settings for my whole family at thanksgiving. It is a motley looking set up!
