We are going to Universal in Oct - and now fancy going to Mythos for a meal. Do I need to book in advance before our trip and if so how far ahead?

No. If you're staying on property, they usually allow priority seating by showing your room key. If you're not staying on property, stop by guest services near the entrance of IOA when you arrive at the park, and make a reservation at that time.

We've been going to (and LOVING!) Mytho's at least twice on all of our trips since 2005, and have never had a wait. I always make reservations just because I find it fun planning everything out in advance, but it's never been crowded on all of our trips. It really isn't a neccessity.
