Service name is just a repeat of user name (if you want it to be). It's the name that shows up when you post on forums. Hope that helps.
As far as QR vs SR...I prefer QR. Their surveys pay more. 15 cents more for Opinion, 25 cents more for OTX and GiveUsYour2cents. 65 cents doesn't sound like much, but it adds up.
I'm coming up on $200 in Disney Gift cards from QR. Usually takes about 5 days to get them once I cash in.
I love em and recommend them.
