I think it's probably only in the Catholic church that that's really done. We're Lutheran (my dad is a pastor with 40 years of experience) and I don't think he's ever gotten a monetary fee for baptisms. Like PP, it's generally part of the normal Sunday service.
For ours, we gave gifts to the congregation in the form of hiring special musicians for the service (classic Spanish guitar for one and a bagpiper for the other) and then gave gifts to the godparents (Waterford crosses.)
