It is my understanding that the Guest Medical Information Form is mainly used to determine whether there is a basis for a handicapped accessible stateroom request, or whether there is a possibility of pregnancy that falls within the prohibited cruising rules. The ADA prohibits medical information from being used as a method of screening out people with disabilities and DCL is required to follow the guidelines based on ADA court decisions.
I take medication, hate to reveal its nature, but recognize that as I travel alone, if I should become really sick or badly injured it might be a good idea for someone to have some kind of idea what they may need to take into consideration before pumping me full of other drugs.
You don't need to fill out a form for medicine that is prescribed, but make sure you carry them in their labeled bottles.

The medical form is basically a request for special services, whether they are dietary, room accomadations, or whatever. If you are an adult who takes you medication on you own or administers it to your children, they ask only that you bring it in the original, labelled containers. There is no need to do the form.
