Medical form DCL

I just know you can help me out here. :) I've just booked my first disney cruise. :thumbsup2 Because I need (and got) a handicapped accessible stateroom, I need to fill out the medical form. Ok, no problem. Just one question though; what do they want you to fill out at the 'current medical status' part? I can think of a couple of options.

- just needs
- a diagnosis
- a combination of both of the above
- option 3 + detailed info about medical status and all of those 'has a problem with.....' 'such and so might happen'.

I'm off to my GP in an hour (didn't expect to get an appointment so quickly :scared1: ) and am wondering what he should put in there. I know we should inform them about the powerchair and needing a rollin shower (hence, handicapped accessible stateroom), but what else?

Only a diagnosis, ok no prob. But option 4? That's like a laundry list of small and bigger stuff caused by that one main diagnosis. We'll need like 5 more lists to fill that out. :scared1: Those are things that I'm certain of being able to handle on a cruise, as I do in every day live. I know where I'll need some help or a certain aid and will inform disney of this. For instance, I'm due to have a stoma soon. No big thing, but a potential risk (most will be internal, most medical professionals will not notice me having it. So some heads up on what to take care of in the event that I can't is neccessairy) if not taken care of in the unlikely event of me being unconcious for whatever reason. So of course I'll inform DCL of that after the operation, for the 'just in case'.

But do I need to inform them of the rest? ( I don't want to keep things behind, but on the other hand don't want to 'hand out' my medical file to 'anyone').
 
my GP gave my diagnosis and medication details on the form itself and then he did a covering letter for any other relevant info he felt they needed to know, the box is extremely small and if you have a long list i would ask if your doctor can do the same

we go in may and i will try to report back on facilities for you
 
What I did was give some basic information on the form and then marked "see attchment"

On the attachment I had four parts to it.

My prescribed medications including dosage and when or how often I took them.

My over-the-counter medications which my doctor had advised mne to take, including dosage and when or how often I took them.

A brief list of my medical conditions

What I felt my needs were.

I then attached the list to the form and submitted it.

We decided to do it the 'easy yet sure way'. For now we've put down my current medication (which we know will change), my current needs and current diagnosis. We know stuff will be added to this, so we'll just send in a second form (with a copy of the one we'll be sending in now and telling which still is accurate and what not) a month or 2 before the actual sail date to update them on the changes.
