Medical form and pregnancy

I will be 17 weeks when we sail in May. Do I need to request the medical form or is it generally just sent with the cruise documents?

No. We'll be on the May 18th Wonder! Congratulations!! Glad to know there will be other people who will look like me on the ship...you know not really big enough for mat clothes but too big for regular clothes...Gotta love that point in pregnancy
