I have not gotten my documents yet, is there a voucher for each? quest riding? is there one thats lists everyone? What information is on the for such as DOB name etc...?
do they confirm each quest against their photo ID?

I would give Disney a call and see if they in fact have you listed for DME. Or you can call DME directly...don't have the number handy, but it's here on the board someplace. Someone should have it.
If you don't get your info packet, with vouchers, before you travel, and you do have reservations for DME, you can just go right to the DME counter and they will print you out vouchers for the bus. Yes, each person riding the bus will need a voucher. While they don't cross check with ID's, they will see x number of people listed on the voucher, and only that many people will be allowed on the bus.
If, again, you don't get your packet with tags/vouchers in time, you can either go to baggage claim yourself and get your checked bags and bring them with you, or you can give the claim receipts to DME people at the checkin desk and they will send someone to baggage claim to get them for you. If they send someone to get the bags, they will in turn transport the bags to your room. If you get the bags yourself, you will have the bags with you..they will put them underneath your bus.

We get one voucher for the entire party. It lists only the name of each guest and the flight information. No photo ID is required to board DME if you have your vouchers. Not sure what they might require if you had a reservation but no vouchers.
 

The name of each guest using the bus. If you don't get vouchers in the mail, then the CMs at the checkin counter will print them up for you....each person listed on the room reservation according to their system.
I'm not sure why you keep asking about dob or addresses. Are you trying to replace one person with another, that might not be listed on the reservation?
If you have listed 4 people on the room reservation and also reserved DME, then you will get spots for 4 people. I have never had ID's checked when boarding a bus. You could feasibly have different people than those listed ride the bus....basically, any combination of people, as long as you stay within the number of people listed with DME/Disney.

A three year old will need to be on your room reservation (and also have their own airplane seat), so she will have her own DME reservation and can sit in a seat or on your lap.
