Lottery syndicate contract ?

Has anybody devise a lottery syndicate certificate that they could kindly share please?

We've downloaded a syndicate manager tool off the National Lottery Website and been using it for a couple of years. It has a contract on there but also you can record whos paid and it will also check the numbers for you!

I tried to download 3 times.But no joy.Looks like ill have to send off for disc.
But am off to Florida on Thur for 3 weeks and wanted to get it sorted before I go - so if anybody has a copy of a contract I could personalise please,pretty please
