Looking for room request e-mail form/link for Dolphin

Hi - I used the e-mail room request form for the Dolphin this past July and had everything as I requested waiting for us when I checked-in. My boss is going next week and I am trying to find the e-mail form/link for him to use, but I can't find it. Can someone help me locate it. Thanks.
