Looking for Personalized"livestrong" style ID bracelet

I remember seeing these posted in the past, but can't find anything now.

I'm looking for those rubber bracelets (like the yellow Livestrong ones) that you can have personalized with your cell phone number as an ID for a child.

We have an engraved tag we attach to DS pants in the parks, but I would love these for the water park.

I've found some sites to order in bulk, but we only need 1-2.
