Looking for Kids daily WDW diary?

Our School has aked our kids (8 and 6) to compleate a daily diary whilst on holiday. I had found such a thing with individual sheets for each day pre printed that the kids could use but i am afraid i lost it. Anyone got a link to such a diary?
