I am pretty sure you have to take care of this in person. It also takes 8-12 weeks for your tile to be mounted.

Since they have to take your picture, it is somewhat difficult to get a tile without actually being at Epcot.

There are 3 of us, the pictures can only be 2, so we opted for just our names and dates. Many did not have photos.
