Doesn't your dentist do a pre-treatment plan? When I take the kids in for their 6 month cleaning/check-up, if they need any work done the dentist does a pre-treatment estimate so I know the charges and they send it to the insurance company to make sure it's covered. And then I've had them put on our files that I do not authorize any treatment not covered by insurance unless they talk to me first. So we haven't had the problem for a while now.
