I had both of the things mentioned above. And you don't get bread at lunch (at least we didn't-I don't know if it is a "if you ask for it, you can get some" type thing).
Have eaten several times at Kona, most recently October only for dinner though and always been PLEASED! Have also read that the bread is not served at lunch.ENJOY!
