My son who just turned 4 when we went and got an ear infection on the ship. He was miserable the last night of the cruise and we brought him to the doctor on the ship and they were wonderful. Would we have cancelled if we knew he would get sick? Yeah but did it ruin his trip? Nope. Actually he did bounce back quick but be warned they do not like it when kids fly with an ear infection so after our cruise it was lucky we had 6 nights booked at WDW. It gave him plenty of time to clear up.
