We are planning a surprise WDW for DD in November. It was sudden and within 90 days so I thought Chef Mickey's was out of the question. I've been calling for 2 weeks and finally got the reservation I wanted. We are going to get to go to MK the first day we are there, watch Wishes at 6pm, and then go eat with Mickey and friends at CM the first day we are there. All the other Wishes times are at 9pm which is way past DD's bedtime, so I am so glad it worked out!!
If you are trying to get the ADR you want, please don't give up. Magic happens you just have to keep trying and not give up.

If you're staying on site, when you make your reservation be sure to mention that she'll be celebrating a b'day. They may add some extra pixie dust when you're there. I'd also mention that at check in. The last 2 years they have given my dd an autographed picture of her favorite character wishing her a happy birthday, and also given her some balloons. This isn't guaranteed, but it doesn't hurt to let them know and see what happens.
Also, they offer birthday buttons for free at Guest Services in each of the 4 theme parks.
