How to swap ADR's?

So, if we've found someone to swap reservations with, how do we go about doing it? Do we just give the other person the confirmation number, and they give us theirs, or do we have to go through the official Disney channels, or what?

I never swapped, but I did give away a few reservations. I just PM'd with the person who wanted the reservation. They gave me their name and phone number.

I called Disney Dining and gave them my confirmation number, and changed the name and phone number on the reservation to that persons name and gave their phone number.

It worked great, and there was no confusion on the CM's part in doing this.
