How often do you wash and replace your comforter/duvet cover?

I wash my sheets and pillow cases weekly but usually only wash the comforter about twice a month. However, we're terrible about kicking aside the top sheet and usually end up sleeping directly under the comforter, so I'm wondering if I ought to wash it more.

Also, how often do you replace your comforter or duvet cover with a new one? They wear so gradually that it's hard to tell when it starts to actually look worn out.

Wash it every couple of months (we use a top-sheet). The last time I replaced one was when we redecorated; the previous one would have been 4 years old then and it wasn't worn out.
 
We use a top sheet so was the comforter about every other month. Now that we have two little kittens we started putting a blanket over the comforter during the day, easier to wash the blanket weekly
 
I just replaced my duvet cover and comforter (insert), it had been a while since we had replaced it, I was waiting until we decided how to redecorate the room but realized we were not any closer to a decision and bought something I liked but the old cover was still in great shape. I wash the duvet and insert every month or so, a little more often in the summer as we don't use a top sheet.
 
Every couple of months. I used to do it a little more often but I've been slacking a bit. Did the sheets every week but now I'm going about every two weeks.
 
Every other week at least. My bf refuses to use a top sheet + pets
It's about time for a one tho. He's had it since before I knew him and we've been together over 4 years now
 
I change sheets once a week and we have a top sheet. I change the bedspread every season and depending on the spread, most of the time it gets washed before it is put up. I do have a couple of quilts that were hand made by my Granny that can not be washed that often as they are over 100 years old, those are actually used in the upstairs bedrooms that don't get slept sometimes at all so they just get put away at the change of the season. I have a heavy winter bedspread for our bed that has to be dry cleaned and I'll admit I only take it about every 3 or 4 years but there is always a sheet and or a blanket between us and that.
 
Duvet covers red washed weekly. Dogs...
 
We don't have a duvet cover, just sheets, blankets, a duvet, and a coverlet. The whole bed is entirely white, and I take everything off every Sunday and wash it in hot water with bleach. I like a clean bed. :)
 
I'm trying to get into the habit of at least every other week, but preferably once a week. We just moved into a place that has laundry in unit and am trying to be better about washing things frequently. We only sleep with the comforter and have a dog in bed with us. I slacked a lot on washing them before - lugging 3 loads of laundry to and from parents' houses meant laundry got done maybe monthly.
 
THE sheets get changed weekly, but we have a top sheet and blanket, so the comforter or bedspread gets wash when it needs it. I keep a blanket on the bottom of the bed for the cats and just wash that a few times a month. My comforter or bedspread gets changed out by season, so it is never on the bed long. I have Summer, Fall, Christmas, WInter, and Spring. I get bored.
 
I wash the sheets and duvet cover every week or so. No top sheet. Now the actual duvet that I only wash once or twice a year or so but I usually buy a new one about every three years.
