My husband and I are in the process of remodeling a <b>30-year old</b> house, and ordered custom vanities thru Home Depot several weeks ago. They are coming to install them TOMORROW!!
I assume when you ask "how hard is it" - that you're intending to do the job yourself. Well - Home Depot and Lowe's both have many styles of "standard-size" vanities that you can purchase. And, if you don't want to do it yourself - they will come out and install it. The pre-made cabinets are quite reasonably-priced, IMHO.
As far as the custom cabinets. We went in and sat down with a designer; looked at the different cabinet manufactures, different door styles, different wood species, and different stains. He has a computer program that will lay out the cabinets on the screen, and make all changes necesssary until you're satisfied. They came out to the house to measure, then we went back in and finalized our purchase. Then - they even came back out for a second measure - just to be sure.
Depending upon the countertop you choose - it may be contructed based on the cabinet measurements, or they may have to come out and create a template once the cabinets are installed, then return with the top after it's been constructed.
We ordered Thomasville cabinetry for both bathrooms. In the master, we went with a granite countertop; in the guest, we went with Corian. (FWIW. We originally chose a "Stonehouse" countertop for the guest but, when it came in, we were not satisfied with the color. Home Depot took it back - no hassle AT ALL!)

Oh - just wanted to say we also went to a "bathroom and kitchen designer" before going to Home Depot. Their quote was MUCH more than we ended up paying - and we are thrilled with our cabinets! The kitchen comes next....
