How do you pay for gratuities and any extras when doing DDP?

Last time we did the DDP gratuities were included (that really made the plan a bargain!!!). How does it work on the new plan? Can you just leave cash tip or do they have to bill it to your card? Most meals we are a party of 5 but inlaws are joining us for 2-3 meals (but they are not on the DDP) so those will be for 7.

Also, If we are ordering something not included in the plan do they give us a separate bill for those items at the end of the meal or does that get charged to the card as well??

Just trying to figure out how it all works and how much cash I need to plan for food!

We have used DDP the last few years, and always used cash. It's still accepted more places than any other card. When I get American cash at the bank for our trip I make sure to get quite a few smaller bills to make tipping easier.
Always ends up being a $16-$18 tip at most places for 3 of us.
