Help with video intro?

I have this video intro that someone on here made for me about a year ago and i used it for the intro for our dvd slideshow --
i would love to do the same for our trip last week, but a spring break intro won't work.
Does anyone have or know where to find a blank/generic version of this?
I did check the original thread, but the 4shared account is inactive.
