Go to www.amymickey.com and order her transfers. I had never made transfer T-shirts before, but these were so easy to use that I had no problems making them. They have also lasted through multiple washes without cracking or fading!
I have used Avery, Wilton and Amy Mickey transfers. Avery & Wilton cracked and faded after one hand washing & line dry. Amy Mickeys are dark so you have to plan your design accordingly but they are worth it - several launderings in the washer and dryer and not a crack or fade. They are totally worth the extra cost and planning!!!
I used them on a range of colors... from white to black. Check out others' finished product photos. Obviously some background colors look better on different shirts. Let me know if you would like me to e-mail some pics of what we did.

We bought our shirts from jiffyshirts.com. They were fast and have a huge selection for all sizes. Amy's transfers are dark, so you don't have to mess wth reversing the images. If you are putting them on a lighter colored or white shirt, you may want to put a border around your design. I put mine in MS Word and added borders like this:
