Help With Disney Survey

HELP!!!

I received a postcard to participate in a Disney survey, and the web address doesn't work!!!

How should I contact Disney to let them know that the site is down?

They might be having trouble with their server. I'd just try back later, or tomorrow. I get those surveys and sometimes wait like 2 months to do them.
