Help with cruise documents...

Can someone help me with filling out my cruise docs? Specifically the one related to authorizing charges. We will be two adults and two kids. I assume I do nothing with the ones in the kids docs, but does just one adult need fill it out and then list the other adult and kids since they will have charging privs?

Also, we are on the 06/01 Magic and there is a Bahamas imigration form -- is that for Castaway Cay or do I just leave it blank?
