help with cruise documents

What all needs to be filled out? I am the credit card holder whose name the reservations are under. I have 2 minor children who will not have charging priveleges. On their signature forms, do I just sign for them or sign my name or what? I just want everything to be in order so there are no hold ups at registration.

the way that I did it was to sign for him, and initial. Then when we boarded i went directly to guest services and made sure that his KTTW was not activated.

I am sure someone else will post how they have handled this.
 
on the immigration form our kids actually signed it. :D

On the credit card form we left it blank.

On the contract an adult have to sign on behalf of the minor.

Did I get that right??
 
I signed the credit card form for them, with my name and a note "for dd/ds name" That's the way we sign government documents when the supervisor/signor isn't there, so that's why I did it that way. I also didn't list their names on the "who's authorized to charge" line. BUT it does say that you have to visit guest services to ensure no charging priveleges are on the card.

On the contract, there is a spot for adult signing for minor (or other incapable party). You sign in the indicated spot.
