Help! Shipping Box to AKV

I am getting my OL this trip so I have several things I need to take down. I need to ship a box down. Can you please give me the correct address to ship to AKV? I have seen two different ones and I am not sure which is correct. Can you also tell me everything that is required on the label? Thanks a bunch! 1 week from tomorrow I will finally be back in my happy place
