Help me fix my ptr :)

I have 2 questions.

1. How do I create links to each of my posts individually. I want to have a chapter listing but I cant figure out how to link to just the post.

2. How do I change the name of a link. I have noticed that when people do chapter listings, they don't list the whole address, just the name of each chapter as the link. Does that make sense??
