How many of you actually had to use this form?? I called DCL and they said that they don't ask for it only some destinations do. (But you know how many CM's tell you different things) Ie.... Mexico
I asked my Lawyer about this and she had never heard of it. I emailed her MANY cut and paste about this letter and again, she had never heard of it and she has been a Family Law Attorney for 25 yrs.
We are in the process of me getting Sole Parental Responsibility so it shouldn't be an issue come next May but you never know. Florida is a hard state to get this in.

With my sons' passport, we are having to sign the parental consent form that the post office has, stating that his bio father is unable to be located, thus his signature can not be obtained.
We have been talking about passports for my kids and the "letter" that is needed from the absent parent for that!!!
He has placement custody, but, the mom is a nut in all sense of the word and we know she will not sign the form...so sad for children in this situation...again, thank you for leading me to the form..Hmmmmm..... I did not know that!!! But the thing is we know where he is and how to contact him. My luck I would get caught and it would be ME in trouble!!!

WOW!!! This is GREAT to know!!! Thank you SOOOOO much for this info!!! We are going to court to have him ordered to sign the form for the passport. I KNOW he will not do this. (out of spite) At least NOW I know, I can take that court order about the passport and get the girls passports.
I am still VERY concerned about taking them to places like Mexico because Mexico is where I read that they DO require a letter from the absent parent. If FL doesn't give me Sole Parental Responsibility then I am screwed and stuck with taking the girls to the Bahamas all of the time when we cruise.
But no worries, there are plenty of other places they can go!!!
