Help finding old post--name tag template

Hello-
I remember reading a post that I thought I'd never forget--it may have been a sticky...the poster left specific instructions on how to make your own classic CM name tag door magnet. He included a link and info on printing it out. Does this sound familiar?

I would like to print out a couple of DCL tags personalizing them with my kids' names. It sounded very easy to do and I don't really want to bother a disigner for something I can do myself.

Can anyone point me in the right direction? I've tried the search option and I'm not getting the result I need.
