Help!!! Does anyone have the WL email for concierge

Hi all!!! I just upgraded to Wl concierge but I am about 92 days out. Does anyone have a copy of the email they send for you to fill out for your requests or the email address for the WL concierge. Need to get this to them ASAP since they havent contacted me and I want to make sure I get my ps's 90 days out. THANKS so much!!!!

we used it for the concierge in Feb.
