Have I left anything out of this note granting Sisinlaw permission to act as parent?

My kids will be spending a week with my Sisterinlaw in another state this summer. During that time they will spend 2 hours per day taking horseback riding lessons. I want to ensure that my sisterinlaw & her husband can perform all acts that normally require parental authority. I have drafted the following note to the camp. Please let me know if you can think of anything else I need to include. (I will be doing a separate noterized medical authorization form for non-camp time and giving SiL a copy of both boys' insurance cards.)

To Whom It May Concern:

My children, DS1 and DS2, will be under the care of their aunt, DSiL during the August 2004 camp session. I authorize DSil or her DH to:

1- transport my children to and from camp location(s);
2- grant any and all permissions normally requiring parental signature;
3- authorize any medical treatment he/she deems appropriate; and
4- any & all other acts requiring parental
permission/authorization.

don't forget to give them your DS's insurance cards if they have them. Our insurance gives each person their own card. (It's a pain).


You might want to have the letters noterized (sp). It depends on the laws of your state and the state they will be in (if another). Call your insurance company just to be sure.

SiL will absolutely have copy of insurance card with notorized letter which will also indicate that DH & I will be responsible for any medical costs not covered by insurance.
 
Not sure if it's this way everywhere, but I used to work as a registration clerk in an ER, and we couldn't accept a medical authorization from a parent unless it was notarized. You may want to include your phone number if you didn't, I assume you wouldn't post it if you did of course, but in case of an accident or something like that, couldn't hurt.

If they ran into a problem, needing medical attention. Consent can be obtained over the phone by 2 RN's. I think this should be adquate for the camp.
