Do I need to fill this out for all members of my party or just for someone with a medical condition? All of them need to be signed by a Dr.? How soon can I get them filled out if I have to--like is October too early for an April cruise?
 
You only need the medical forms if someone has a medical condition.
 
Hope you don't mind me riding piggy back on your thread.

I have allergies but I don't want to spend the $90 for an office visit plus the $10 per page fee to fill out my medical form, could I just list the allergies without the doctors signature?
 
Kind of what I thought--so my son with Asthma and allergies is a yes, and the rest of us don't need one!
 

My understanding is that you would only need the form filled out and a Dr.'s signature if you require special accomodations.

We needed a refrigerator for meds. DCL also supplied us with a sharps container, which was a nice touch.

I'm really not sure about the allergies. I would probably err on the side of caution and get the form signed though.

I know not every medical office has the same policies, but I was able to drop the form off and have it filled out/signed without paying for an office visit. It's just one sheet. Our Dr. did not charge anything to sign the form, but I'm sure there are some who do.
