Guest Assurance Card, can someone give me details

I'm not asking for myself, we're going to Disney next month on a Make a Wish trip, but I'm asking for a friend who's 5 year old son has some mobility/stamina issues.

How/Where do you get one?

Do you need proof of a medical condition to get it?

What does it do exactly for someone that is using it?

I know they are planning to stay at a Monorail Resort because he can't walk long distances without getting worn out.
