Many folks visiting WDW have no doubt already thought about this; but in case there are a few who never gave it a thought ...

Create a concise medical record for yourself and each member of your family. Make it all fit on one page for each person, and let the first line advise: “THIS RECORD COMPILED BY FAMILY” Near the top of the page, include the birth date, home address, home phone number, and any person’s phone number you would want notified in the event of a major medical incident.

Include any major medical events in your life (serious illnesses, surgeries, etc.), indicating the date of each occurrence. (Do not include events that are not likely to be of importance in any future medical emergency. You want to keep this page as neat and brief as possible, but helpful in the event of an emergency.)

Also include the name, address, and phone number of your primary family doctor; your health insurance companies’ address, and phone number to verify coverage; and any allergies, including any adverse reactions to medications.

List all prescriptions currently being used, being sure to include the dosage.

Add any information that you think it would be necessary for an emergency responder to know. But remember to keep it concise, brief, and to the point.

Hopefully, you will never need the info page you have created for each family member. But in the event of a real emergency, you will be very glad you took the time to record this info.

And, if never needed, you will still reap the benefit of knowing you are prepared, and making every preparation possible to help your loved ones. Adds a layer to your peace of mind, don’t you know.

You may think, “Why should I write all this stuff down? I know it by heart.” Perhaps so, but in the frighting moments during a real medical emergency it is often impossible to remember all the important information those responding might need to know.

If this ends up helping even one person, I'll be glad I took the time to write this post.

Excellent advice, you never know what may happen.
I had a seizure in a computer lab during college--because I was by myself and just had my research materials and no id...the hospital labled me "jane doe" and wouldn't give me any meds until I was coherent enough to confirm who I was and call my parents who were hours away and my roomate who happened to be my Db (now Dh)...this was years ago and it still bothers me.

Always good to have medical history/info on hand.

Also, hopefully this vital information won't get lost - because you've put all the important ID-stealing info in one neat page...
