I finally got to the link and filled in all the info except that it requires a 'campaign code' which I can't find.

Maybe they changed it because it didn't ask for that when I filled it in. I just found a site that said to use code bxhedjcz. When I tried using that code it thanked me for my previous request so hopefully it will work.
