I read somewhere on her (can't find it now though) that you can buy Jif Peanut Butter and get a free Disney Scrapbook Kit. I printed out the order form but now that I read it closer it says "no copies, facsimiles, or reproduction of the Official Mail Order Form. So where can I get an original form?

I believe they are referring to the reciept that you need to include when they say no copies, etc.

I just sent off for this today..and sent the form from the website with original store receipt. It sure looks nice!
