Fort Wilderness fax form (to request specific loop)

I posted this under the "Disney Resorts" forum, but thought I'd post here as well.
I read in another thread that I must fax my request for a specific loop directly to FW and that there is a form floating around somewhere on the DIS. I've done several "search forum" searches, but I keep coming up empty. Does anyone know where this request form might be located?

I usually go to the post office or my accountant for tax forms. Oh - FAX forms - my bad.
