Flying - Confused with Fed. Attestation form

So the form asks you to fill out all the people in your party. Then it says everyone over 16 has to sign.

Does that mean that everyone on the list has to have a copy of that 2 page document and each sign it or does just one person in the travel party sign for everyone.

Why ask who all you're travelling with if you all have to have an individual form?

If you are going to be flying, you fill out an attestation form as part of your online check-in for each person, it isn't a separate form. You don't have to fill anything else in. Nobody asks to see the attestation form.

Nope. Nobody asked us to see any attestation form and the questions on the form are the same as the ones on the online check-in.

This is basically the questionnaire you fill in on your check-in form. I think the form you posted is out of date.

Well, it's the one that Air Canada sent us today for our upcoming flight, so I was just trying to do what I'm supposed to do. Everything is confusing these days.

Ah that must be new since the rules changed for flying within Canada!

The attestation we filled out online was for the US gov't where the one you are linked looks like it is for the CAN gov't. We left on Dec. 1 and I guess that wasn't in place yet.

Sorry, I am no help whatsoever for that form!

In 4 recent trips to the US (there now) I have never been asked for my signed form. That said, I did print out 5 copies (1 for each of us) for our recent flight. My thought was better safe than sorry!
 
That's what the paperwork says... you probably won't need it, but if you are asked, you have to provide it. Way to kill trees! 2 pages for each person for each flight.
 
I was asked to show my mine when I flew. As were those checking in around me (an Air Canada agent was checking as you approached the check-in area).
 
Also, in case it is helpful and you have little under the age of 12, for the “testing on day 3-5” if asked (I have not been) you can say you plan to do the Bimax Now antigen test from CVS/Walgreens, etc. They are super easy (and affordable, why can‘t we have these in Canada!?!) Again, never been asked, but just in case.… We actually used one today for my eldest who has a sniffly nose (because he didn’t listen to his mother and wear his coat on a rainy 40 degree day!!?!) It was negative, mom is always right! ;)
 
Didn't they just announce they're going back to PCR tests for everyone no matter how long you're out of the country and it has to be taken outside of Canada?
 
Oh, I thought you had to carry a print out with you, since it has to be signed.
That's what I thought. I had the form filled out and ready, no one asked to see it. Westjets online check in only required me to read it and acknowledge that I had read it.

Much better to have it but not need it than not have it an need it.

I have multiple versions of eveything (in the app (I have the Swiss COVID certificate app - the QR code doesn’t work here, but the details can be read from it; files on my phone; and paper, as available).

ETA - that wasn’t meant to be a lecture to Adam’s mom. Just thoughts using her post as a jump of point.
