Filling in Docs -- Questions for Those Who Have Filled Them in Before

We got our docs yesterday and started filling them out. For the most part, things are pretty self-explanatory, but I have a few questions. We're doing the 4 day Wonder cruise, so we must fill out the Bahamanian entrance form. On it, there is a section asking where we will be staying and for how many nights. Do we just put 0 for nights? And there's no place for "cruise ship" for "Where are you staying?" (One choice is private boat, but the Wonder is hardly a private boat.) Do we check "other"?

Also, in the individual booklets that we fill out, on the form that is for your card / charges, do I put only the name of the person who owns the credit card where it says "card holder"? This form is obviously to document charges to the KTW cards. All three of our cards will be on one credit card which is held in my name. Do I put my name at the top of all three of these forms? (It's not very clear. On the one hand, I can see where it might be the name of the holder of the KTTW card. On the other hand, it makes more sense that "cardholder" refers to the person who holds the credit card.)

Is that clear as mud? Anyone???

On the Magic now. Folks at the terminal helped me fill out that pesky Bahamian form. You can put 'PC" in the departure city, and the Date for when you visit Castaway Cay for the entry date.

I just filled out one form for the state room account. They do a swipe of a credit card while checking your docs.
