I'm trying to think through what I use the most. Here are some:
Flash (I like Canon 580 EX II) - the more the better
software (I like Lightroom and PS CS5)
bubble level
remote shutter release
white balance aid (card or expodisc)
memory cards (I like having a pair of fast 32 gig cards)
backup battery
polarizing filter
variable ND filter
cleaning stuff for lens and sensor
portrait grip and a hand strap
L-bracket
spare body cap
photo hosting site (I like Smugmug)
video hosting site (I like Vimeo)
printer (I like Epson or Canon)
hoodman loupe if you shoot a lot outside
small flashlight if you shoot at night
camera strap (I like BlackRapid for singles and OpTech for duals)
camera bag
Lots and lots of lenses. And still more lenses. And just a few more. And maybe one more after that.
If you shoot video, either an external audio recorder or something like a Videomic. A white balancing aid is even more important with video. An external monitor (I like SmallHD), although the articulating screen on the 60D makes that somewhat less necessary.
There's a few, but my most used and must haves are the Black Rapid strap and a Lens Pen.
I have some friends that swear by the Luma Loop strap, which I have to admit seems nicer than the Black Rapid. Either way, there is no way I would use a standard strap to carry a camera for any length of time. With the sling straps even my heaviest lenses are nothing to carry all day long.
I keep forgetting about how much i like my external flash, as well. I have a Yongnuo YN465 (very inexpensive, and still great!!!!) But almost any external flash will completely blow away the on-camera one.

A camera strap and a screen protector for the LCD were the first things I bought when I had my Canon. It came with a vertical/battery grip which was one of my favorites accessories too.
the first thing I always buy when i get a new DSLR is the proper battery grip. The cameras are just to small for me to hold well without one. The next accessory I always take with me is an external flash. There is so much more you can do with one over the built-in. In fact, if the camera came without one I wouldn't miss it...
