I understand how FP works. I think I understand how baby swap works. What I don't get is how to make the 2 work together. For example say the 4 of us get FPs for ToT. When the time comes, DH takes the 2 oldest while I keep the little one entertained. When they are finished, we have only 1 FP left. Or will we get the baby swap FP even if they used the FP to ride (meaning we really only need to get 3 FPs from the machines)?

Before they go on the ride ask the CM for a rider swap pass, When you go back on the ride you go up the fastpass entrance and you can take 2other guests with you for a total of 3 riders. Almost all rides have the rider swap pass. The only ride we used it that didnt have fastpasses was goofys rollercoaster, the cm gave me the fast pass and i was told to just walk up the exit to the ride, we had no trouble at all. So you really can just get 3 fastpasses bc when you ask for the riderswap its basically a fast pass. And you dont have to go right back on with the riderswap, you can come back later that day if you like.

Yes, the rider switch pass is basically a fast pass that can be used by up to 3 people.

As the other poster mentioned, make sure you ask for the rider switch pass before the first group gets in line (you will have to show that you have baby/small child to get one). The first 3 ride with their fastpasses. Then the second group rides with the rider switch pass (using the fastpass entrance). Using fastpass and rider switch together is the best way to go. Neither my DH nor I ever had to wait more than 10 minutes or so with the baby, so we didn't have to split up throughout the day.

Really gory details for Rider Switch are available on my Fastpass page here.

The Rider Switch pass is good for up to three guests, so if you are using it the older children would get to ride TWICE - they'll love it.

Note that in order to get an RS pass, your youngest MUST be under the height limit. Some CMs may be flexible, but the rule is that its a height thing.

Using FP and RS together is a bit confusing, depending on the size of the family and the CM at the Fastpass line. Some insist that everyone have a Fastpass if the first group is using the Fastpass line - then you trade one FP in for the RS pass. Others don't really care.

In general, we've used the RS pass with the regular Standby line and used FPs for attractions we can all enjoy.
